The gastrofiberscopy as well as the roentgenologic examination of the stomach is now considered to be one of the indispensable diagnostic method of gastric diseases.
For evauation of the gastrofiberscopy in the diagnosis of varous gastric diseases, the diagnostic accuracy was investigated in 597 cases who underwent the fiberscopic examination during the period of August 1, 1971 through December 31, 1976.
Of 288 cases of gastric carcinoma, a type of gastic carcinoma was specified under the designatioof carcinoma with lymphoid stroma, A histologic feature distinguishing this carcinoma was the presence of aa nondesmoplastic stroma infiltrated uniformly with an abundance of lymphocytes and plasma cells throughout the entire area of the tumor,
The incidence, gross finding, and survival time of this gastric carcinomas were examined.
The following results were obtained.
1. Sex distribution of 597 - cases male 432 cases (72.4%), female 165(27.6%) .sex ratio, 2.61:1. Age distribution second decade 26 cases (4.0/61, third 75(12.6%), fourth 162(27.1%), fifth 201(33. 6%, sixth 104(17.5%), and seventh 29(4.8%).
2. Each fiberscopic diagnosis in the 597 cases was classified as follows :the normal stomach 67 cases (11.2%), gastric ulcer 64(10.7%),gastritis 115(21.0%), gastric cancer 276 (46.2%). Postoperative condition 17(2.8%) and failure to examination 16(2.7%).
3. The diagnostic accuracy of fiberscopic examination was 93.8%(2701288) in gastric cancer, 90.49%(66/73), in gastric ulcer and 86.8%(125/144 in gastritis
4, Of the 288 cases of the advanced .gastric cancer, the incidence of gastric carcinoma with lymphoid stroma was 18 cases (6.4%), of which 14 cases Were dead and 4 cases alive. The median survival time of the 14 dead cases was 25 monthsin comparison with 8 months of control group and the duration of the 4 survival cases was. from 11 months to 7.5 years as of the date.
